Welcome to the Inform Formulary

Work is ongoing to ensure the Formulary reflects practice across the whole Health Board following the merger with the Bridgend Locality in April 2019

This INFORM formulary lists all medicines approved for use in Primary and Secondary care in Cwm Taf Morgannwg UHB, together with the relevant prescribing categories, prescribing notes and links to useful sources, and is managed by the Cwm Taf Morgannwg UHB Medicines Management Practice Unit. 
Chapters are subject to ongoing review but
MHRA warnings and related prescribing information may not be included; SmPCs and other sources should be consulted for current prescribing information, together with relevant guidance before making therapeutic decisions.

Non formulary products: See the non formulary product flowchart, available here, to determine the appropriate process to be followed. Non formulary forms are available form the pharmacy department.

Dietary products: Not generally listed on Inform. Click section A2 - Borderline substances (BNF tree) and refer to Dietetic Formularies
Wound care products: click section A5 - Wound management (BNF tree) to see individual documents listed for details
